The short answer

Short studies in healthy people suggest that repeated higher-dose aspirin can affect gastric or measured vitamin C status, and vitamin C changed some gastric or duodenal injury measures. These findings support considering adequate intake in people with poor intake or repeated higher-dose exposure. They do not establish clinical vitamin C deficiency, routine testing, megadose treatment, or prevention of major bleeding or ulcers.

Measured vitamin C change was short term

In 45 healthy subjects, the initial regimen of aspirin 0.8 g three times daily lasted six days. The aspirin-only group had lower gastric-mucosal, plasma, and urine ascorbate at day 7, and the authors describe about a 10% gastric-mucosal reduction. The protocol later switched treatments without washout, but the abstract does not state a fixed second six-day exposure period.

Gastric findings are not all the same

One three-day volunteer study reported that vitamin C attenuated aspirin-associated gastric damage with aspirin 400 mg twice daily and vitamin C 480 mg twice daily. Another used aspirin 900 mg twice daily and vitamin C 1 g twice daily. In that crossover study, vitamin C did not reduce any gastric-injury parameter, while it reduced duodenal injury by Lanza score.

What these studies cannot promise

None of the reviewed studies establishes clinical vitamin C deficiency, routine laboratory testing, or an aspirin regimen for everyone. They also do not show that vitamin C prevents major gastrointestinal bleeding, replaces evidence-based ulcer treatment, or makes higher-dose aspirin safe. Their short duration and healthy-volunteer populations set clear limits.

What this means for use

For someone with poor intake or repeated higher-dose aspirin exposure, adequate dietary vitamin C is a reasonable topic to discuss with a clinician. Do not treat the study doses as a recommended consumer dose. The current low-dose aspirin label names black stools, vomiting blood, persistent stomach pain, and faintness as warning signs that need urgent medical evaluation.
