Vitamin E from food should not automatically be treated like a high-dose capsule. NIH ODS describes different natural and synthetic alpha-tocopherol forms and different IU-to-milligram relationships. Many vitamin-E-only supplements contain amounts far above ordinary dietary intake. The reported 0.4 g (400 IU)/day notation in one older trial is preserved as published, not converted into a general dose equivalence.

In a six-week volunteer study, vitamin E plus aspirin was associated with lower platelet adhesion to collagen, while aspirin alone did not affect that measure. Platelet testing is not the same as a clinical major-bleeding outcome, but it gives a reason to take high-dose supplement use seriously when aspirin is already part of a bleeding-risk picture.
The clinical bleeding estimate remains uncertain
In the 100-person preliminary TIA study, hemorrhagic stroke incidence did not differ significantly, although both events occurred in vitamin E recipients. In the 4,495-person Primary Prevention Project, severe bleeding was 1.1% with aspirin versus 0.3% with no aspirin. That was an aspirin main effect, not an estimate of bleeding added by vitamin E.
No prevention or timing promise
The Primary Prevention Project found no vitamin E effect on its prespecified endpoints, so these older studies do not support taking vitamin E for cardiovascular prevention.
