The short answer

Sodium bicarbonate may reduce amantadine clearance under conditions that alter urine chemistry, so regular use needs prescriber review. A small human study found reduced clearance but did not show significantly higher plasma concentrations or prove toxicity.

What this answer covers

Sodium bicarbonate oral products versus intravenous bicarbonate studied experimentally; dose, duration, kidney function and amantadine release form matter.

Supplement or preparation
Sodium bicarbonate
Medication ingredient
amantadine

The current label recognizes a clearance concern

GOCOVRI labeling warns that a shift toward alkaline urine can allow amantadine accumulation and calls for monitoring adverse reactions. This is a renal-elimination concern, not evidence that bicarbonate increases intestinal absorption.

The human experiment had a negative blood-level result

A small randomized crossover thesis studied healthy men given oral amantadine syrup and intravenous bicarbonate or saline. Kidney clearance fell, but measured plasma concentrations were not significantly higher. The study did not establish clinical toxicity.

Route and formulation limit the extrapolation

The original work was a master’s thesis rather than a peer-reviewed journal article. Intravenous bicarbonate loading is not ordinary oral antacid use, and amantadine syrup is not GOCOVRI extended-release. Those boundaries prevent a fixed oral-supplement effect estimate.

Review repeated bicarbonate use

Tell the prescriber about regular bicarbonate use, the exact amantadine formulation and any kidney problems. Report new confusion, hallucinations or marked drowsiness. Do not adjust amantadine or rely on a short timing gap to prevent a renal-clearance interaction.

Evidence and practical considerations

Side effects & toxicity

Conditional accumulation/adverse-effect concern from current GOCOVRI urine-pH label and human IV-bicarbonate clearance finding. Thesis plasma concentrations were not significantly higher and no toxicity was proven; oral bicarbonate and IR/ER effect sizes not established.

Exact scope
Sodium bicarbonate oral products versus intravenous bicarbonate studied experimentally; dose, duration, kidney function and amantadine release form matter. with Oral amantadine hydrochloride immediate-release capsules; GOCOVRI extended-release considered separately.
Medication form and route
oral medication; oral supplement

Population: People prescribed amantadine

May cause insomnia, agitation or somnolence; careful observation with CNS stimulants and other CNS-active medicines. Renal elimination and seizure-history precautions.

How this applies: Exact amantadine RxCUI 620 IN, with stated salt/formulation/route bridge.

No exact supplement-combination incidence or validated protective spacing; salt, route and release form limits apply.

Population: People prescribed amantadine

GOCOVRI extended-release amantadine hydrochloride: urine alkalinization may reduce excretion and allow accumulation; monitor adverse reactions. Somnolence and concomitant sedating medicine precautions. Not interchangeable with other amantadine products.

How this applies: Exact amantadine RxCUI 620 IN, with stated salt/formulation/route bridge.

Same active ingredient, different formulation or combination. Product-specific results cannot be transferred as identical exposure.

Population: 12 healthy adult men in a randomized two-arm crossover.

Renal clearance decreased and estimated half-life increased, but no significant treatment effect on measured plasma concentrations was found. Authors infer reduced tubular secretion; exact mechanism unresolved.

How this applies: Direct substances with route/formulation limits; clinical oral-supplement risk remains conditional.

Small short study, IV bicarbonate and oral amantadine syrup, not chronic oral baking soda or GOCOVRI. Not peer-reviewed; no demonstrated toxicity or significant plasma-level rise.
