The short answer

Tolmetin’s label recommends antacids other than sodium bicarbonate. Ask your pharmacist about an alternative; an exact absorption change or protective spacing interval is not established.

What this answer covers

Sodium bicarbonate used as an oral antacid with tolmetin sodium capsules containing tolmetin-equivalent doses. Dietary bicarbonate and other antacids are separate.

Supplement or preparation
Sodium bicarbonate
Medication ingredient
tolmetin

The label names sodium bicarbonate specifically

For GI symptoms, the tolmetin label recommends antacids other than sodium bicarbonate. Ask the pharmacist to choose an appropriate option instead of assuming all antacids are interchangeable. The label does not provide a protective spacing interval.

A different antacid had different evidence

A crossover study tested aluminum and magnesium hydroxide with tolmetin sodium and found no meaningful change in most pharmacokinetic measures. That does not establish a similar result for sodium bicarbonate, which is a different ingredient.

Do not infer a precise absorption change

The inspected evidence does not establish how much sodium bicarbonate changes tolmetin levels or effectiveness. Findings involving tolfenamic acid, a different drug, cannot be used to supply that missing answer. Do not adjust the prescribed tolmetin dose to compensate for an assumed effect.

Persistent stomach symptoms need review

Antacids should not conceal persistent pain or possible bleeding during tolmetin treatment. Contact the prescriber about continuing symptoms and seek urgent care for vomiting blood or black, tarry stools. Review the reason for bicarbonate if it was medically prescribed.

Evidence and practical considerations

Absorption & effectiveness

The direction and extent of a sodium bicarbonate effect on tolmetin absorption are not established by the inspected evidence.

Exact scope
Sodium bicarbonate used as an oral antacid with tolmetin sodium capsules containing tolmetin-equivalent doses. Dietary bicarbonate and other antacids are separate. with Exact tolmetin IN 10636 linked by current RxNorm to tolmetin sodium PIN 42935. Capsules contain 246 mg sodium dihydrate equivalent to 200 mg tolmetin. Other NSAIDs are not interchangeable.
Medication form and route
Oral

What remains uncertain

  • Original antacid trial tested aluminum/magnesium hydroxide, not sodium bicarbonate.

Factors that may matter: Exact preparation and dose; Fasting versus fed dosing; Bleeding and ulcer history; Other medicines.

Research conclusion: insufficient evidence · Evidence assessment: low
