Two labels support the concern

The lithium oral solution label says alkalinizing agents can lower lithium levels through increased urinary excretion. The sodium bicarbonate capsule information also gives a lithium-specific warning about increased excretion and reduced plasma levels.

The concern is not established poor absorption

The bicarbonate warning attributes the lithium effect to kidney handling. It does not quantify the effect of each dose or demonstrate failure of lithium citrate absorption. Amount, duration and clinical context therefore matter.

Antacid spacing has a different purpose
