The short answer

Nopal can change meal-related glucose responses, but added hypoglycemia with insulin aspart protamine has not been demonstrated. Steamed food and dried-capsule findings differ.

What this answer covers

Opuntia ficus-indica steamed plant material versus dried capsules or concentrated extracts; medication scope is fixed NovoLog Mix70/30.

Supplement or preparation
Opuntia ficus-indica
Medication ingredient
insulin aspart protamine, human

A meal study found a glucose effect

Steamed nopal lowered the glucose response to a high-carbohydrate breakfast in a small diabetes study. Meal composition affected the response; this was not an exact premix-interaction study.

Capsule results were less clear

Another study found no acute glucose or glucose-tolerance effect from dried capsules. Usual glucose-lowering drugs were withdrawn in its diabetic crossover arm, so that arm was not combination treatment evidence.

Do not turn an average change into an event rate

Neither abstract identifies this premix or establishes a rate of clinically low glucose. Food and concentrated supplements cannot be assumed equivalent from the species name.

Review major food changes too

Discuss the exact supplement and amount with the diabetes team and follow your glucose-monitoring and low-glucose treatment plan. Do not change the premix dose yourself. Large dietary changes can matter to the meal plan as well as concentrated supplements.

Evidence and practical considerations

Side effects & toxicity

Preparation-specific meal effects and contrary capsule findings do not establish clinical hypoglycemia with the exact premix.

Exact scope
Opuntia ficus-indica steamed plant material versus dried capsules or concentrated extracts; medication scope is fixed NovoLog Mix70/30. with Insulin aspart protamine human RxCUI352385 IN as part of NovoLog Mix70/30, SC only.
Medication form and route
subcutaneous insulin premix; oral supplement

What remains uncertain

  • Combination label cannot isolate protamine component; no exact herb-event rate or universal adjustment.

Factors that may matter: Fixed component ratio; Meals; Preparation; Dose; Other glucose-active treatment.

Research conclusion: insufficient evidence · Evidence assessment: very low

Population: Adults prescribed the fixed insulin-aspart premix

Hypoglycemia can be severe; niacin can reduce glucose lowering. Fixed ratio does not permit independent basal/prandial adjustment. Dose within15minutes before meal start; type2 patients may also dose after meal start as labeled.

How this applies: Exact RxCUI352385 IN insulin aspart protamine, human, addressed as component of this labeled fixed combination.

Combination-product evidence cannot isolate the protamine component or represent pure soluble aspart, NPH or other premixes. No herb-specific event rate.

pubmed.ncbi.nlm.nih.gov · Source check Sep 10, 2026

Population: 14type 2 diabetes participants; separate index study in7healthy people.

Steamed nopal lowered post-meal glucose response with a high-carbohydrate breakfast; meal composition affected responses.

How this applies: Indirect for insulin aspart protamine as part of NovoLog Mix70/30; original preparation, drug and endpoint retained.

Small meal experiment; no exact insulin-aspart-protamine co-use or clinical hypoglycemia analysis in abstract. Food preparation differs from extracts.

Source 3: original human study

pubmed.ncbi.nlm.nih.gov · Source check Sep 10, 2026

Population: 10diabetic acute participants,10healthy OGTT participants,14diabetic crossover participants and5healthy participants.

Dried nopal capsules showed no acute glucose-lowering effect or OGTT effect; chronic comparison largely reflected glucose worsening during placebo.

How this applies: Indirect for insulin aspart protamine as part of NovoLog Mix70/30; original preparation, drug and endpoint retained.

Small studies; oral hypoglycemic treatment withdrawn in diabetic crossover arm, so not combination evidence.
