The short answer

Gymnema can change glucose measures and insulin requirements, but added clinical hypoglycemia with insulin aspart protamine has not been established. The inspected insulin study does not identify this premix.

What this answer covers

Gymnema sylvestre leaf extract GS4; other powders, extracts and blends differ. NovoLog Mix70/30 contains two insulin components.

Supplement or preparation
Gymnema sylvestre
Medication ingredient
insulin aspart protamine, human

An insulin study gives a reason for review

A small study added GS4 leaf extract to insulin and reported lower insulin requirements and glucose measures. This suggests a possible treatment effect that warrants review when use changes.

The exact insulin was not established

The inspected abstract does not identify insulin aspart protamine or a specific premix. It also does not establish a rate of clinical hypoglycemia. Lower insulin requirements cannot be converted into a universal dose reduction.

An extract result does not define every product

The study tested a particular water-soluble leaf extract. Its findings cannot predict every Gymnema product, and its proposed explanation involving endogenous insulin is not proof of pancreatic regeneration.

Use the individual monitoring plan

Discuss the exact supplement and amount with the diabetes team and follow your glucose-monitoring and low-glucose treatment plan. Do not change the premix dose yourself. No protective herb-spacing interval follows from the study.

Evidence and practical considerations

Side effects & toxicity

An insulin study shows glucose/requirement changes but does not verify the exact premix or establish added clinical hypoglycemia.

Exact scope
Gymnema sylvestre leaf extract GS4; other powders, extracts and blends differ. NovoLog Mix70/30 contains two insulin components. with Insulin aspart protamine human RxCUI352385 IN as part of NovoLog Mix70/30, SC only.
Medication form and route
subcutaneous insulin premix; oral supplement

What remains uncertain

  • Combination label cannot isolate protamine component; no exact herb-event rate or universal adjustment.

Factors that may matter: Fixed component ratio; Meals; Preparation; Dose; Other glucose-active treatment.

Research conclusion: insufficient evidence · Evidence assessment: very low

Population: Adults prescribed the fixed insulin-aspart premix

Hypoglycemia can be severe; niacin can reduce glucose lowering. Fixed ratio does not permit independent basal/prandial adjustment. Dose within15minutes before meal start; type2 patients may also dose after meal start as labeled.

How this applies: Exact RxCUI352385 IN insulin aspart protamine, human, addressed as component of this labeled fixed combination.

Combination-product evidence cannot isolate the protamine component or represent pure soluble aspart, NPH or other premixes. No herb-specific event rate.

Source 2: original human study

pubmed.ncbi.nlm.nih.gov · Source check Sep 10, 2026

Population: 27insulin-dependent diabetes patients given GS4.

Insulin needs and glucose measures decreased with GS4; glycosylated measures remained above controls.

How this applies: Indirect for insulin aspart protamine as part of NovoLog Mix70/30; original preparation, drug and endpoint retained.

Abstract does not identify this insulin premix, establish hypoglycemia incidence or test spacing. Proposed beta-cell recovery remains a hypothesis.
