The short answer

Opuntia ficus-indica may affect glucose control. Review the product before combining it with linagliptin; the exact added low-blood-sugar risk has not been measured.

What this answer covers

Nopal pads or stem preparations, with fruit and other cactus products excluded from automatic equivalence.

Supplement or preparation
Opuntia ficus-indica
Medication ingredient
linagliptin

What the human evidence shows

Steamed nopal with a high-carbohydrate meal lowered the glucose response in a small diabetes study. A dehydrated-stem study did not lower fasting glucose in participants with diabetes. Neither established risk with linagliptin.

Why your treatment context matters

Linagliptin particularly increases low-blood-sugar risk when combined with insulin or insulin-releasing medicines. It should not be assumed to behave like those medicines by itself. The prescriber should consider the full regimen, meals and prior low readings before deciding whether additional monitoring is useful.

Check the supplement preparation

Identify whether the product is a pad, stem, fruit or extract and record the amount. Meal composition and preparation affected the original results.

Keep the prescribed medicine plan

Continue the prescribed linagliptin dose and review the complete medicine and supplement list with the pharmacist. A new supplement is not a reason to adjust this medicine yourself or invent a separation interval.

Watch for low blood sugar

Follow your existing glucose-monitoring and low-blood-sugar treatment plan. New sweating, shakiness, hunger or confusion warrants a glucose check when possible. Severe confusion, a seizure or inability to swallow safely needs emergency help.

Evidence and practical considerations

Side effects & toxicity

Conditional glucose-monitoring concern follows from supplement glucose effects and the exact medicine context. This is not direct evidence of increased hypoglycemia or altered drug absorption.

Exact scope
Nopal pads or stem preparations, with fruit and other cactus products excluded from automatic equivalence. with Exact linagliptin single-ingredient oral tablets. Exact RxNorm IN1100699 linagliptin; single-ingredient oral tablets. Combination medicines are not assumed equivalent.
Medication form and route
oral
Timing or duration
Source-specific single-dose, repeated-dose and observational exposures distinguished.

What remains uncertain

  • No exact pair human interaction trial identified. Preparation-specific positive and null results do not establish a universal effect, safe dose, spacing rule or drug concentration change.

Factors that may matter: preparation and plant part; clinical indication; dose; other medicines; baseline glucose and nutrient status.

Source 1: current product label

dailymed.nlm.nih.gov · Source check Sep 11, 2026

Population: People prescribed linagliptin.

Glucose-dependent DPP4 action; increased hypoglycemia with insulin/secretagogues. Strong P-gp/CYP3A4 inducers reduce exposure; CYP3A4 inhibition does not automatically imply a clinically relevant change.

How this applies: Exact RxNorm IN1100699 linagliptin; single-ingredient oral tablets. Combination medicines are not assumed equivalent.

No universal supplement dose, interval, event rate or untested formulation equivalence.

Source 2: original human study

pubmed.ncbi.nlm.nih.gov · Source check Sep 11, 2026

Population: Fourteen adults with type 2 diabetes in meal comparison; separate healthy-participant index study.

Glucose AUC after a high-carbohydrate breakfast was lower with nopal; responses depended on the meal.

How this applies: Only the named drug, preparation, route and measured endpoints apply.

No linagliptin-specific co-use or hypoglycemia estimate. Stem/pad food is not automatically fruit, capsule or extract.

Source 3: original human study

pubmed.ncbi.nlm.nih.gov · Source check Sep 11, 2026

Population: Six adults with diabetes and six healthy volunteers in separate experiments.

No acute fasting-glucose reduction in the diabetes group; glucose-load response was attenuated in healthy volunteers.

How this applies: Only the named drug, preparation, route and measured endpoints apply.

Small acute study with different preparation and population; no linagliptin.
