The short answer

Meal effects depend on preparation

A small study found lower post-meal glucose with steamed nopal in a high-carbohydrate breakfast. Responses differed with the meal composition. This is a food experiment, not a measured regular-insulin interaction.

Capsule findings were less clear

A separate study found no acute glucose or glucose-tolerance effect from dried nopal capsules. In its diabetic crossover arm, usual glucose-lowering medicine was withdrawn, so it cannot establish the safety or harm of combination use.

Glucose lowering does not prove hypoglycemia

Neither inspected abstract provides an exact regular-insulin co-use analysis or a rate of clinically low glucose. The species name also cannot make concentrated extracts equivalent to the studied steamed plant or capsules.

Keep the diabetes team informed

Discuss the exact product and amount with the diabetes team before starting or changing it. Follow your established glucose-monitoring and low-glucose treatment plan; do not adjust insulin yourself. Discuss major dietary changes as well as supplements; do not replace insulin with nopal.
