The short answer
Panax ginseng’s glucose effects vary, and extra hypoglycemia with insulin aspart protamine has not been established. Asian ginseng studies cannot be replaced with findings from American ginseng.
What this answer covers
Asian Panax ginseng root and specific extracts, not every ginseng species or preparation. Fixed NovoLog Mix70/30 insulin scope.
- Supplement or preparation
- Panax ginseng
- Medication ingredient
- insulin aspart protamine, human
Some findings were null or opposite
Acute Asian-ginseng studies did not improve overall glucose or insulin responses; one pooled two-hour glucose measure was higher. This argues against assuming a uniform glucose-lowering effect.
One extract improved a diabetes measure
A short study of vinegar-extracted ginseng reported HbA1c improvement in drug-naive patients. It did not test insulin aspart protamine or establish an interaction event rate.
Processing and species limit transfer
The contrasting results do not establish a safe premix combination dose. Asian and American ginseng are different species, and processing changes the exposure.
Keep insulin and supplement decisions coordinated
Discuss the exact supplement and amount with the diabetes team and follow your glucose-monitoring and low-glucose treatment plan. Do not change the premix dose yourself. The current label covers both insulin components; it does not support independent adjustment of the protamine fraction.
Evidence and practical considerations
Side effects & toxicity
Variable preparation-specific glucose findings without verified exact premix co-use do not establish clinical hypoglycemia.
- Exact scope
- Asian Panax ginseng root and specific extracts, not every ginseng species or preparation. Fixed NovoLog Mix70/30 insulin scope. with Insulin aspart protamine human RxCUI352385 IN as part of NovoLog Mix70/30, SC only.
- Medication form and route
- subcutaneous insulin premix; oral supplement
What remains uncertain
- Combination label cannot isolate protamine component; no exact herb-event rate or universal adjustment.
Factors that may matter: Fixed component ratio; Meals; Preparation; Dose; Other glucose-active treatment.

Population: Adults prescribed the fixed insulin-aspart premix
Hypoglycemia can be severe; niacin can reduce glucose lowering. Fixed ratio does not permit independent basal/prandial adjustment. Dose within15minutes before meal start; type2 patients may also dose after meal start as labeled.
How this applies: Exact RxCUI352385 IN insulin aspart protamine, human, addressed as component of this labeled fixed combination.
Combination-product evidence cannot isolate the protamine component or represent pure soluble aspart, NPH or other premixes. No herb-specific event rate.

Population: Two groups of11healthy volunteers.
No significant overall glucose/insulin effect; pooled Asian-ginseng treatment had higher two-hour glucose than placebo.
How this applies: Indirect for insulin aspart protamine as part of NovoLog Mix70/30; original preparation, drug and endpoint retained.
Small healthy-volunteer studies, not insulin-premix co-use. American-ginseng findings cannot be automatically transferred to Panax ginseng.
Source 3: original human study
pubmed.ncbi.nlm.nih.gov · Source check Sep 10, 2026
Population: 72drug-naive adults with type 2 diabetes.
Vinegar-extracted Panax ginseng improved HbA1c in a small trial, with no severe adverse events reported in the abstract.
How this applies: Indirect for insulin aspart protamine as part of NovoLog Mix70/30; original preparation, drug and endpoint retained.
Drug-naive participants, not insulin-premix co-use. Specialized extract not interchangeable with all root products.
