The short answer

Sodium citrate may lower lithium levels when used in amounts that act as an alkalinizing treatment. The concern is increased urinary lithium removal, and its size has not been measured for every product.

What this answer covers

Oral sodium citrate; supporting label combines sodium citrate dihydrate with citric acid. Food-additive exposure is not automatically medicinal alkali exposure.

Supplement or preparation
Sodium citrate
Medication ingredient
lithium citrate

A kidney-clearance concern

Sodium citrate can provide an alkaline load. The lithium solution label warns that alkalinizing agents can increase urinary lithium excretion and lower lithium concentrations. This supports a conditional concern about blood levels rather than a demonstrated reduction in gut absorption.

How directly this has been studied

The supporting sodium citrate label explains the product’s alkalinizing action, but does not report a lithium citrate coadministration experiment. The amount, formulation, kidney function and reason for treatment matter. A warning about medicinal alkali use cannot quantify every supplement or incidental food exposure.

Spacing does not resolve the mechanism

No reviewed source establishes that taking these products a few hours apart prevents the renal interaction. Nor does the lithium label require that every necessary alkalinizing treatment be abandoned. A clinician can assess whether a level check or treatment adjustment is appropriate.

Plan changes with the prescriber

Give the treating team the exact product and daily amount before starting, stopping or substantially changing it. If it is prescribed for a kidney or acid-base condition, coordinate changes with that clinician. Do not adjust lithium yourself to compensate for a possible interaction.

Evidence and practical considerations

Absorption & effectiveness

Conditional alkali-class extrapolation from sodium citrate combination label; no exact standalone citrate dose-response or absorption study.

Exact scope
Oral sodium citrate; supporting label combines sodium citrate dihydrate with citric acid. Food-additive exposure is not automatically medicinal alkali exposure. with Lithium citrate oral solution, exact RxCUI 52105; current NLM IN. Carbonate evidence is indirect through lithium ion where stated.
Medication form and route
oral lithium citrate; oral supplement

What remains uncertain

  • Source-specific identity, preparation and endpoint limitations apply; no unstudied dose conversion.

Factors that may matter: Exact preparation; Amount and changes in use; Kidney function; Other medicines.

Research conclusion: supported with conditions · Evidence assessment: low

Population: People prescribed lithium oral solution.

Lithium is eliminated mainly in urine. Sodium depletion can reduce clearance and raise levels; xanthines and alkalinizing agents can increase urinary excretion and lower levels. Serotonergic agents, explicitly including St John’s wort, increase serotonin-syndrome risk.

How this applies: Only the stated identity, preparation, route and endpoints; limitations also govern extrapolation to the exact pair.

Lithium active-moiety bridge supports renal pharmacology across salts but not identical formulation kinetics, dose equivalence for self-use or quantified effects of every herb.

dailymed.nlm.nih.gov · Source check Sep 10, 2026

Population: Users of the specified oral preparation.

Sodium citrate is metabolized to sodium bicarbonate; the formulation acts as a systemic alkalinizer.

How this applies: Only the stated identity, preparation, route and endpoints; limitations also govern extrapolation to the exact pair.

Manufacturer label for an unapproved-drug-other listing, not FDA approval. Combination formulation is indirect for standalone sodium citrate; no lithium coadministration experiment.
