The short answer

Bitter melon may affect glucose control. Review the product before combining it with linagliptin; the exact added low-blood-sugar risk has not been measured.

What this answer covers

Momordica charantia; capsule, fruit and standardized extracts vary.

Supplement or preparation
Momordica charantia
Medication ingredient
linagliptin

What the human evidence shows

A small type 2 diabetes capsule trial found no significant overall glucose benefit. A separate prediabetes extract study found some post-challenge improvements. Neither established this exact combination’s hypoglycemia risk.

Why your treatment context matters

Linagliptin particularly increases low-blood-sugar risk when combined with insulin or insulin-releasing medicines. It should not be assumed to behave like those medicines by itself. The prescriber should consider the full regimen, meals and prior low readings before deciding whether additional monitoring is useful.

Check the supplement preparation

Food portions and concentrated capsules can differ substantially. Bring the species, plant part and extract dose to the pharmacist rather than assuming every bitter melon preparation has the same effect.

Keep the prescribed medicine plan

Continue the prescribed linagliptin dose and review the complete medicine and supplement list with the pharmacist. A new supplement is not a reason to adjust this medicine yourself or invent a separation interval.

Watch for low blood sugar

Follow your existing glucose-monitoring and low-blood-sugar treatment plan. New sweating, shakiness, hunger or confusion warrants a glucose check when possible. Severe confusion, a seizure or inability to swallow safely needs emergency help.

Evidence and practical considerations

Side effects & toxicity

Conditional glucose-monitoring concern follows from supplement glucose effects and the exact medicine context. This is not direct evidence of increased hypoglycemia or altered drug absorption.

Exact scope
Momordica charantia; capsule, fruit and standardized extracts vary. with Exact linagliptin single-ingredient oral tablets. Exact RxNorm IN1100699 linagliptin; single-ingredient oral tablets. Combination medicines are not assumed equivalent.
Medication form and route

Timing or duration
Source-specific single-dose, repeated-dose and observational exposures distinguished.

What remains uncertain

  • No exact pair human interaction trial identified. Preparation-specific positive and null results do not establish a universal effect, safe dose, spacing rule or drug concentration change.

Factors that may matter: preparation and plant part; clinical indication; dose; other medicines; baseline glucose and nutrient status.

Population: People prescribed linagliptin.

Glucose-dependent DPP4 action; increased hypoglycemia with insulin/secretagogues. Strong P-gp/CYP3A4 inducers reduce exposure; CYP3A4 inhibition does not automatically imply a clinically relevant change.

How this applies: Exact RxNorm IN1100699 linagliptin; single-ingredient oral tablets. Combination medicines are not assumed equivalent.

No universal supplement dose, interval, event rate or untested formulation equivalence.

Source 2: Original human study

pubmed.ncbi.nlm.nih.gov · Source check Sep 11, 2026

Population: 40 adults with type 2 diabetes on standard treatment.

No significant benefit in HbA1c, fasting glucose or several other metabolic outcomes; a few mild adverse events occurred.

How this applies: Only the named original drug, preparation, route and endpoint apply; no untested exact medication transfer.

Small trial; original abstract does not identify linagliptin co-use. Product findings are not proof about every preparation.

Source 3: Original human clinical study

pmc.ncbi.nlm.nih.gov · Source check Sep 11, 2026

Population: 76 participants with prediabetes randomized; glucose-affecting medicines excluded.

Some post-challenge glucose and glucagon measures improved.

How this applies: Only the named original drug, preparation, route and endpoint apply; no untested exact medication transfer.

Selected untreated population, product-specific findings and multiple glucose measures. This was not a linagliptin combination trial.
