The combination has been used deliberately
A small randomized trial after breast radiation used pentoxifylline 400 mg three times daily plus vitamin E 400 IU daily for six months. Investigators reported good tolerability and studied tissue fibrosis. This counters a blanket claim that the two must never be used together; it does not prove every dose safe.
The trial cannot rule out uncommon bleeding
The registry reports no serious adverse events in either group, but the study was small and used a threshold for reporting other events. Its enrollment and duration differ slightly from the journal report. Neither source was designed to establish a precise rate of rare bleeding for all pentoxifylline users.
High supplement doses still deserve review
NIH guidance notes bleeding concerns with large vitamin E doses and medicines that affect clotting. Pentoxifylline also carries bleeding precautions. Tell your clinician the dose, formulation and any aspirin or anticoagulant use; the evidence does not give a universal safe cutoff for this combination.
Follow the clinician’s intended regimen
If the two were prescribed together after radiation, ask that clinician before changing the plan. If you are adding vitamin E yourself, discuss why it is needed and what monitoring fits your history. A few hours of separation is not an established way to prevent bleeding, and dietary vitamin E is a different exposure from high-dose supplements.
