The short answer

Horsetail may increase urine output, but the available short human trial did not show significant sodium or potassium loss and did not include lithium users. A lithium citrate toxicity interaction remains unconfirmed.

What the human study measured

Thirty-six healthy men received a standardized Equisetum arvense extract, placebo and hydrochlorothiazide in separate four-day periods. The extract increased negative fluid balance, but urinary sodium and potassium did not change significantly.

Why the distinction matters

The lithium label warns that diuretics causing sodium loss can reduce lithium clearance. A fluid-balance effect alone does not prove the same sodium loss or lithium interaction. No lithium concentrations or treatment outcomes were measured in the horsetail trial.

Keep species and exposure separate

The trial used a particular aerial dry extract for only four days in healthy men. It cannot establish long-term safety, effects in kidney disease, or the effects of other horsetail species and mixed urinary products.

Review meaningful fluid changes

Lithium levels are sensitive to dehydration and changes in salt balance. Tell the treating team about significant diarrhea, sweating, poor fluid intake or a new urinary product. Do not alter the lithium dose yourself. A fixed gap between doses has not been shown to prevent changes in kidney clearance.

Lithium is eliminated mainly in urine. Sodium depletion can reduce clearance and raise levels; xanthines and alkalinizing agents can increase urinary excretion and lower levels. Serotonergic agents, explicitly including St John’s wort, increase serotonin-syndrome risk.
