The short answer

A specific SAMe interaction with asenapine is not established. Discuss the supplement with your mental-health prescriber before adding it.

What this answer covers

Oral S-adenosylmethionine supplement; concentrated mood product distinct from ordinary dietary nutrients.

Supplement or preparation
S-Adenosylmethionine
Medication ingredient
asenapine

Mood history matters

NCCIH cautions that SAMe may worsen mania in people with bipolar disorder. This is a supplement concern, not proof that asenapine causes a new interaction.

The human trial used other medicines

A small SAMe study added it to serotonin-reuptake inhibitors. Its results do not establish the safety or benefit of adding it to asenapine.

Do not convert mechanism into a diagnosis

General advice about serotonin-increasing products does not establish serotonin syndrome with this exact combination. Review all antidepressants and supplements as well as the prescribed antipsychotic.

Keep treatment supervised

Bring the product and your reason for taking it to the prescriber. Do not use SAMe to replace or offset asenapine, and report substantial changes in sleep, energy or behavior.

Evidence and practical considerations

Side effects & toxicity

Exact-pair side-effect claim remains insufficient; psychiatric history and other medicines justify individual review.

Exact scope
Oral S-adenosylmethionine supplement; concentrated mood product distinct from ordinary dietary nutrients. with Exact asenapine maleate sublingual tablets or asenapine transdermal system, distinguished by route. Exact RxNorm IN784649; SAPHRIS maleate sublingual tablet and SECUADO asenapine transdermal system distinguished. Do not assign swallowed-tablet or sublingual food/drink instructions to the patch.
Medication form and route
sublingual; transdermal
Timing or duration
Source-specific single-dose, repeated-dose and observational exposures distinguished.

What remains uncertain

  • No exact combination trial or measured event rate. General serotonin advice cannot be assigned automatically to an antipsychotic.

Factors that may matter: preparation and plant part; clinical indication; dose; other medicines; baseline alertness.

Research conclusion: insufficient evidence · Evidence assessment: low

Population: People prescribed asenapine.

Somnolence, orthostatic and QT precautions. UGT1A4 and CYP1A2 major metabolism; no dose adjustment advised with CYP3A4 inducers based on PK studies. Do not eat or drink for ten minutes after a sublingual dose.

How this applies: Exact asenapine maleate sublingual route; patch instructions require the separate SECUADO label.

No supplement-specific event rate, universal timing rule or untested route/preparation equivalence.

Source 2: official supplement guidance

nccih.nih.gov · Source check Sep 10, 2026

Population: People considering SAMe.

Possible interaction with serotonin-increasing medicines and supplements; bipolar disorder and limited long-term safety warrant review.

How this applies: Supplement-only concern, not a serotonin-syndrome warning in the antipsychotic label.

General supplement psychiatric precautions, not an exact asenapine interaction or incidence.

Source 3: original human study

pubmed.ncbi.nlm.nih.gov · Source check Sep 10, 2026

Population: 73 serotonin reuptake inhibitor nonresponders with major depression.

Response and remission favored SAMe; discontinuation due to adverse events was not significantly different.

How this applies: Other-medicine clinical context only.

Serotonin-reuptake-inhibitor augmentation trial; no asenapine coexposure established.

Source 4: official product label

dailymed.nlm.nih.gov · Source check Sep 10, 2026

Population: Adults with schizophrenia prescribed asenapine patches.

Somnolence, cognitive/motor, orthostatic and QT precautions; clinical interaction data include studies of sublingual asenapine.

How this applies: Exact ingredient in a distinct transdermal formulation; route bridge kept explicit.

No exact supplement interaction study; sublingual food/drink interval cannot be assigned to the patch.
