Can I take Humulus Lupulus with Clozapine?

Review Humulus Lupulus with Clozapine: possible adverse effects, preparation details, evidence limits, and sources.

The short answer

Hops preparations may impair alertness, but an increase in clozapine-related sedation has not been established. The official monograph gives a driving precaution and reports no interactions. That does not prove co-use is safe, especially when a product contains other sleep ingredients.

What this answer covers

Humulus lupulus hop-strobile preparations taken by mouth; beer, isolated constituents and valerian-hops blends are not interchangeable.

Supplement or preparation
Humulus lupulus
Medication ingredient
clozapine

What the official information says

Hops has traditional use for mild stress and sleep. The monograph cautions that it may impair driving and machinery use, while recording no reported interactions. Clozapine itself can cause sedation. This is a reason to pay attention to impairment, not proof that hops increases clozapine toxicity.

The trial measured mood

A small crossover trial in young adults tested a hops dry-extract product for four weeks. Self-reported anxiety, depression and stress improved, but the study did not establish effects on clozapine or an increased sedation risk. A change in mood is not the same endpoint as reduced alertness.

Blends need their own review

A hops-only preparation is different from a valerian-hops sleep product or an alcoholic drink. The presence of other active ingredients means the effects cannot be assigned to hops alone. Show the full product label to the prescribing team rather than relying only on the word hops.

What to do with the uncertainty

Discuss a new sleep preparation before adding it during clozapine treatment. If you feel drowsy or unsteady, avoid driving and hazardous work and contact the prescribing team about troublesome symptoms. The evidence does not establish a dose-spacing interval or justify changing clozapine dosing yourself.

Evidence and practical considerations

Side effects & toxicity

General hops and clozapine impairment precautions support caution about alertness during co-use.

Exact scope
Humulus lupulus hop-strobile preparations taken by mouth; beer, isolated constituents and valerian-hops blends are not interchangeable. with Oral clozapine, RxNorm 2626 IN.
Medication form and route
oral clozapine; oral botanical preparation

What remains uncertain

  • No direct pair trial; monograph lists no reported interactions.

Factors that may matter: exact botanical preparation; other medicines; baseline drowsiness.

Research conclusion: supported with conditions · Evidence assessment: low

Population: People prescribed clozapine.

Clozapine can cause sedation and impaired cognitive or motor performance; somnolence and instability can contribute to falls. Severe hepatotoxicity has been reported.

How this applies: Medication-specific safety context only; no direct botanical interaction or automatic dose change is established.

Does not name any of the six botanical counterparties or quantify their combined risks.

Population: Users of specified oral hop-strobile preparations.

Traditional use for mild stress and sleep; no interactions reported; may impair driving and machinery use.

How this applies: Botanical-only evidence; relevance to clozapine requires separate interpretation.

No clozapine interaction study. A 2025 addendum was read separately; no demonstrated direct clozapine signal was identified.

Side effects & toxicity

The small hops-only mood trial does not establish additive sedation, respiratory depression or a clozapine interaction.

Exact scope
Humulus lupulus hop-strobile preparations taken by mouth; beer, isolated constituents and valerian-hops blends are not interchangeable. with Oral clozapine, RxNorm 2626 IN.
Medication form and route
oral clozapine; oral botanical preparation

What remains uncertain

  • Self-reported mood endpoints; no clozapine group and incompletely characterized extract.

Factors that may matter: exact botanical preparation; other medicines; baseline drowsiness.

Research conclusion: insufficient evidence · Evidence assessment: low

Population: 36 young adults completed; six of 42 did not complete.

Self-reported depression, anxiety and stress scores improved; cortisol and body measures did not significantly change.

How this applies: Botanical-only evidence; relevance to clozapine requires separate interpretation.

Mood outcomes are not sedation or respiratory-depression endpoints. No clozapine use; extract details insufficient for universal product equivalence.
