The direct trial had a narrow setting
Eleven regular cannabis smokers received mirtazapine or placebo in a controlled withdrawal and relapse model. Sleep improved during abstinence, but this does not establish how an oral hemp or CBD product affects daytime alertness.
Reported harms were not all sedation
A separate case review included mirtazapine with THC-rich products and other medicines. Its mirtazapine events were not the sedation events reported for another antidepressant.
Do not assume a shared metabolism effect
CBD minimally affected mirtazapine metabolism in a laboratory assay. A citalopram concentration increase in the same publication was a different clinical experiment and cannot be assigned to mirtazapine.
Watch for impaired alertness
Mirtazapine can affect alertness and coordination. Avoid driving if affected and contact the prescriber if added sleepiness, confusion or unsteadiness appears after a new product. Severe confusion, fainting or inability to wake needs urgent assessment.
