Why smoking changes matter
Smoke constituents can increase activity of an enzyme that helps clear clozapine. When an inducing exposure stops, clozapine levels may rise. The clozapine label explicitly identifies tobacco smoke in this process. A cannabis-related report involved tobacco at the same time, so it cannot show how much cannabis alone contributed.
What the case report found
One person taking clozapine developed confusion and higher drug levels after stopping both tobacco and cannabis smoking. The report included a second person taking olanzapine, a different medicine. These are individual cases, not a trial that measures the risk of cannabis with clozapine.
The preparation changes the question
Smoking cannabis is a different exposure from swallowing hemp oil or purified CBD. The smoking report cannot establish that all cannabis products lower clozapine levels, impair absorption or make treatment less effective. A product name alone does not identify the route or chemical exposure well enough to predict the effect.
Discuss changes with the prescribing team
