The short answer
The proposed garlic–nevirapine interaction has not been established in an exact-pair human study. Findings with other medicines do not reliably predict nevirapine levels or harm; review the actual supplement before use.
What this answer covers
Allium sativum preparations; studied caplets are not all garlic oils, aged extracts or food.
- Supplement or preparation
- Allium sativum
- Medication ingredient
- nevirapine
What the original human studies show
A small human garlic-caplet study found lower saquinavir exposure. Saquinavir is a different HIV medicine, and that result does not establish reduced nevirapine absorption or effectiveness.
Nevirapine has more than one metabolic pathway
Nevirapine is handled by CYP3A and CYP2B6 and induces its own metabolism over time. A CYP3A probe result alone cannot establish the overall change in nevirapine exposure, and a different antiretroviral is not an interchangeable test.
Preparation and outcome limits remain
The studies do not supply a nevirapine dose adjustment, clinical failure rate or safe supplement threshold. A laboratory marker, exposure result and clinical adverse event are different outcomes.
Review the full HIV regimen
Tell the HIV clinician which extract and dose you use. Do not interrupt or alter nevirapine to accommodate a supplement. Its serious liver and skin warnings remain important independently of any unproven herbal interaction.
Evidence and practical considerations
Absorption & effectiveness
No exact nevirapine combination result establishes the original claim. Human other-drug findings and nulls retained; CYP3A/CYP2B6, autoinduction and preparation differences prevent automatic exposure or toxicity transfer.
- Exact scope
- Allium sativum preparations; studied caplets are not all garlic oils, aged extracts or food. with Nevirapine immediate-release oral tablets; extended-release formulation considered separately.
- Medication form and route
- oral medication; oral supplement
What remains uncertain
- No unmeasured pair incidence, blanket class transfer or arbitrary spacing interval.

Population: People prescribed nevirapine
Serious hepatic and skin reactions; metabolism through CYP3A and CYP2B6 with autoinduction. St John’s wort co-use not recommended because of reduced exposure/virologic risk.
How this applies: Exact nevirapine RxCUI 53654 IN, with stated salt/formulation/route bridge.
Exact route and formulation apply. Label precautions do not quantify untested botanical pairs or support arbitrary supplement doses.

Population: People prescribed nevirapine
Shares CYP3A/CYP2B6 metabolism, autoinduction and serious liver/skin precautions. Different release/dosing formulation.

Population: Ten healthy volunteers.
Saquinavir AUC fell51% during studied garlic supplementation.
How this applies: Ten healthy volunteers and saquinavir, not nevirapine. Cannot transfer magnitude, absorption mechanism or HIV-treatment outcome.
