The short answer

Aged garlic extract can lower average blood pressure, but excess hypotension specifically with metolazone is not established. Review your readings and product with the prescriber.

What this answer covers

Oral Allium sativum products with metolazone; available evidence concerns a standardized aged bulb extract.

Supplement or preparation
Allium sativum
Medication ingredient
metolazone

The trial showed average pressure lowering

A12-week trial in people with uncontrolled hypertension found lower average systolic pressure with a standardized aged garlic extract. Some participants used stable antihypertensive medicines. Responses varied, and no metolazone-specific harmful hypotension result was established.

Aged extract is not every garlic product

The trial used aged bulb extract standardized to S-allylcysteine. Raw garlic, garlic oil, food servings and other supplements can differ. Show the pharmacist the exact product and daily amount rather than assuming the same effect from a shared garlic name.

Review low readings or dizziness

Metolazone can lower blood pressure and cause fluid or electrolyte disturbances. Ask whether home pressure records are appropriate when a supplement changes. No fixed metolazone dose reduction or timing interval is established for garlic, and garlic should not replace prescribed treatment.

Report symptoms and changes

Report persistent dizziness, cramps, weakness, palpitations or a major change in swelling or urine output. Illness, dehydration and other medicines can contribute. Severe weakness, fainting or a new irregular heartbeat need prompt assessment. Do not treat symptoms by adding salts or increasing the diuretic yourself.

Evidence and practical considerations

Side effects & toxicity

A harmful low-pressure interaction between garlic and metolazone is not demonstrated by the inspected trial.

What remains uncertain

  • No exact subgroup or adverse hypotension endpoint; supplement average BP lowering is not proven additive clinical harm.

Factors that may matter: Exact preparation and dose; Kidney function; Baseline electrolyte status; Other medicines.
