The short answer

Long-term omeprazole may reduce absorption of food-bound vitamin B12 and sometimes contribute to deficiency. This is different from proving that it blocks every B12 supplement. Testing or replacement should reflect symptoms and other risk factors; low-risk PPI users do not automatically need extra B12 or routine screening solely because they take omeprazole.

What does the direct experiment show?

Ten healthy men took omeprazole 20 or 40 mg daily for two weeks. Absorption of protein-bound B12 decreased in both groups. The experiment was small and short, and it did not establish clinical deficiency, test a standard free B12 tablet, or compare injection schedules. The form of the vitamin is therefore central to interpreting the result.

When does this become a long-term concern?

The current label notes that daily acid suppression over a long period, for example longer than three years, may lead to B12 malabsorption or deficiency. It advises considering the diagnosis when compatible symptoms occur. A change in an absorption test is not the same as a confirmed clinical deficiency, so assessment should use the person’s history and findings.

Does everyone need routine tests or extra B12?

For GERD patients on PPIs who have no other B12-deficiency risk factors, the ACG guideline does not recommend routinely increasing B12 intake or screening levels. That advice does not apply unchanged to someone with a previous low result or other reasons for deficiency. The aim is to identify the people who need assessment rather than add a supplement automatically.

Should B12 be taken several hours later?

The checked evidence does not establish an hourly gap that prevents the long-term effect. If B12 deficiency is diagnosed, discuss the cause, replacement route and follow-up with your clinician. Do not assume the protein-bound absorption result makes every oral supplement ineffective, and do not stop necessary acid treatment without reviewing its indication.
