The short answer
Panax ginseng’s glucose effects vary, and extra hypoglycemia with insulin lispro protamine has not been demonstrated. Species and preparation differences limit predictions.
What this answer covers
Asian Panax ginseng root and extracts, not all ginseng species. Fixed Humalog mixture scope.
- Supplement or preparation
- Panax ginseng
- Medication ingredient
- insulin lispro protamine, human
Some findings were null or opposite
Acute Asian-ginseng studies showed no overall improvement in glucose or insulin responses, with a higher pooled two-hour glucose measure. This challenges a uniform glucose-lowering assumption.
A specific extract improved HbA1c
A short study of vinegar-extracted ginseng in drug-naive diabetes patients reported improved HbA1c. It did not test insulin lispro protamine or a premix interaction.
Do not equate species and extracts
Asian and American ginseng are different species. Processing also changes exposure, so the studied products do not define a safe combination dose for every supplement.
Review product changes
Discuss the exact product and amount with the diabetes team and follow your glucose-monitoring and low-glucose treatment plan. Do not change the premix dose yourself. Neither result proves that this exact combination is harmless.
Evidence and practical considerations
Side effects & toxicity
Variable preparation-specific glucose findings lack exact lispro-premix co-use and clinical hypoglycemia evidence.
- Exact scope
- Asian Panax ginseng root and extracts, not all ginseng species. Fixed Humalog mixture scope. with Insulin lispro protamine human RxCUI314684 IN within HumalogMix75/25 or50/50, SC only.
- Medication form and route
- subcutaneous insulin premix; oral supplement
What remains uncertain
- Whole-product evidence does not isolate protamine component; no exact event rate or universal dose change.
Factors that may matter: Fixed ratio; Meals; Product; Dose; Other diabetes treatment.

Population: Adults prescribed the fixed lispro premix
Hypoglycemia can be severe; niacin can reduce glucose lowering. Fixed ratio precludes separate basal/prandial dose adjustments. Inject within15minutes before a meal as labeled.
How this applies: RxCUI314684 IN insulin lispro protamine, human, as component of specified Humalog mixture.
Whole-product evidence does not isolate protamine component, compare every premix or apply to soluble lispro alone. No herb-specific adverse-event rate.

Population: Two groups of11healthy volunteers.
No significant overall glucose/insulin effect; pooled Asian-ginseng treatment had higher two-hour glucose than placebo.
How this applies: Indirect for insulin lispro protamine within the labeled Humalog mixtures; original preparation and clinical endpoint retained.
Small healthy-participant Asian-ginseng studies, not insulin-premix co-use. American-ginseng findings cannot be automatically transferred.

Population: 72drug-naive adults with type 2 diabetes.
Vinegar-extracted Panax ginseng improved HbA1c in a small trial, with no severe adverse events reported in the abstract.
How this applies: Indirect for insulin lispro protamine within the labeled Humalog mixtures; original preparation and clinical endpoint retained.
Drug-naive participants; no insulin-premix co-use. Specialized extract differs from other ginseng products.
