A large fish-oil trial did not show more bleeding

In a randomized study of 1516 cardiac-surgery patients, a defined EPA/DHA fish-oil regimen did not increase major perioperative bleeding compared with placebo. This challenges a blanket claim that every fish-oil exposure increases bleeding. It did not establish the effect of chronic co-use with nabumetone.

Nabumetone has its own safety profile

Nabumetone is converted in the body to an active metabolite called 6MNA. Its label reports little platelet effect and no bleeding-time change in a short study using 1000 mg daily, but serious GI bleeding and ulcer warnings remain. This profile differs from other NSAIDs and does not prove either safety or harm from a particular supplement. The surgery study is not a reason to use nabumetone around coronary bypass surgery, where NSAIDs are contraindicated.

Identify the actual omega-3 formulation

Check EPA and DHA amounts rather than total oil weight, and tell your pharmacist whether the product is a prescription preparation, a mixed supplement or another fatty acid such as ALA. The fish-oil trial does not establish equivalent effects for purified EPA or every product sold as omega-3.

There is no established special schedule

The inspected evidence does not show that nabumetone changes your nutritional omega-3 requirement or requires a fixed separation interval. If a product was prescribed for a separate condition, ask its prescriber before changing it. Review bleeding history and other medicines, and report unusual bleeding or persistent GI symptoms.
