The short answer

High-dose nicotinic acid can worsen glucose control. Review the form and dose of niacin with the prescriber when taking empagliflozin; an exact combination risk has not been measured.

What this answer covers

Niacin forms and doses distinguished: pharmacologic nicotinic acid is not routine dietary intake or all nicotinamide products.

Supplement or preparation
Niacin
Medication ingredient
empagliflozin

What the human evidence shows

A large trial of extended-release niacin plus laropiprant found more serious disturbances of diabetes control. The combination design does not isolate niacin, but official nutrient guidance also recognizes glucose increases with large nicotinic-acid doses.

Check the actual niacin product

Bring the label showing nicotinic acid versus nicotinamide and the daily amount. A lipid-treatment dose should not be treated as equivalent to the niacin in food or a routine multivitamin.

Review changes in glucose control

Tell the prescriber if readings rise after starting or increasing niacin. Empagliflozin should not be increased on your own to counter this effect. The full diabetes regimen and reason for niacin matter.

Keep the prescribed medicine plan

Keep the prescribed empagliflozin plan. Tell the clinician about illness, reduced food intake, vomiting or diarrhea; this medicine also has dehydration and ketoacidosis precautions. Do not change fluid, salt or medicine doses to compensate for a supplement yourself.

Evidence and practical considerations

Side effects & toxicity

General safety claim narrowed to high-dose nicotinic-acid-related glucose worsening, with exact-pair magnitude unknown.

Exact scope
Niacin forms and doses distinguished: pharmacologic nicotinic acid is not routine dietary intake or all nicotinamide products. with Exact empagliflozin single-ingredient oral tablets. Exact RxNorm IN1545653 empagliflozin; single-ingredient oral tablets. Combination medicines are not assumed equivalent.
Medication form and route
oral
Timing or duration
Source-specific single-dose, repeated-dose and observational exposures distinguished.

What remains uncertain

  • No direct empagliflozin interaction study identified. Niacin/laropiprant trial cannot isolate each component or justify a timing workaround.

Factors that may matter: preparation and plant part; clinical indication; dose; other medicines; baseline glucose and nutrient status.

Population: People prescribed empagliflozin.

Glucosuria and volume-depletion/ketoacidosis precautions; hypoglycemia risk particularly with insulin or secretagogues.

How this applies: Exact RxNorm IN1545653 empagliflozin; single-ingredient oral tablets. Combination medicines are not assumed equivalent.

No universal supplement dose, interval, event rate or untested formulation equivalence.

Source 2: Federal nutrient guidance

ods.od.nih.gov · Source check Sep 11, 2026

Population: People taking antidiabetes medicines and high-dose nicotinic acid.

Large nicotinic-acid doses can raise glucose; monitoring is advised during co-use with antidiabetes medicines.

How this applies: Only the named original drug, preparation, route and endpoint apply; no untested exact medication transfer.

General guidance does not quantify empagliflozin-specific risk or establish a timing workaround.

Source 3: Original human study

pubmed.ncbi.nlm.nih.gov · Source check Sep 11, 2026

Population: 25,673 patients with vascular disease.

Active treatment increased serious disturbances in diabetes control.

How this applies: Only the named original drug, preparation, route and endpoint apply; no untested exact medication transfer.

Combination treatment; does not isolate niacin from laropiprant or establish a empagliflozin-specific rate.
