Keep the timing direction clear
RIFADIN labeling says oral rifampin should be given at least one hour before antacids because absorption may be reduced. This is not a symmetric rule that any one-hour gap works in either direction.
The controlled trial found no change
In a randomized crossover study, an aluminum-magnesium antacid did not significantly change rifampin peak concentration, time to peak or total exposure. The participants were healthy and received single doses, so the result does not cover every product.
Antacid salts and mineral nutrition differ
The study did not establish an effect for every magnesium or aluminum salt, and it did not specifically test magnesium carbonate alone. Food minerals, nutritional replacement and intravenous products should not automatically receive the same warning.
Follow the prescribed product schedule
Show the pharmacist the exact antacid or supplement and follow the oral rifampin timing instruction. Do not change the antibiotic dose. Food timing is a separate issue and should not be confused with an antacid interval.
