The short answer

St. John’s wort may reduce eszopiclone’s effect through enzyme induction. This is an indirect prediction supported by the label’s CYP3A4-inducer warning, not a measured St. John’s wort–eszopiclone effect.

What this answer covers

Hypericum perforatum preparations; hyperforin content and repeated-use duration vary.

Supplement or preparation
Hypericum perforatum
Medication ingredient
eszopiclone

Why a reduced effect is plausible

LUNESTA labeling says CYP3A4 inducers may lower eszopiclone exposure and effects. Human St. John’s wort studies with another CYP3A substrate show induction after repeated use, supporting a conditional concern.

The label’s original experiment used a different drug form

The rifampicin experiment described in the label measured racemic zopiclone. The label expects a similar effect for eszopiclone, its S-isomer. That is an explicit pharmacologic bridge, not an exact eszopiclone–herb trial, and its numerical reduction should not be assigned to this pair.

Not every herbal preparation has the same effect

A low-hyperforin St. John’s wort preparation produced a small midazolam change that investigators considered not clinically relevant. Other preparations caused larger changes. This variability prevents a fixed prediction for an unidentified product.

Review starting and stopping the herb

Discuss the product with the prescriber if sleep benefit changes or before starting or stopping it. Do not take extra eszopiclone to compensate. Reduced metabolic exposure is different from proven reduced intestinal absorption, and a short timing gap is not an established fix.

Evidence and practical considerations

Absorption & effectiveness

Conditional reduced exposure/effectiveness from label-recognized CYP3A4 induction and human St John’s wort probe studies, explicitly indirect. The label’s rifampicin experiment was racemic zopiclone, not exact eszopiclone or herb co-use. No established reduced gut absorption, fixed effect size or protective spacing.

Exact scope
Hypericum perforatum preparations; hyperforin content and repeated-use duration vary. with Eszopiclone461016 IN; oral LUNESTA tablets, distinct from racemic zopiclone.
Medication form and route
oral medication; oral supplement

What remains uncertain

  • No unmeasured pair incidence, blanket class transfer or arbitrary spacing interval.

Factors that may matter: Preparation; Dose; Other medicines; Organ function.

Population: People prescribed eszopiclone

CYP3A4/2E1 metabolism; ketoconazole increased exposure2.2fold. Rifampicin induction evidence measured racemic zopiclone, extrapolated by label. Additive CNS depression and boxed complex sleep behavior warning.

How this applies: Exact eszopiclone RxCUI 461016 IN, with stated salt/formulation/route bridge.

Other-drug results do not establish exact botanical effects; single-dose lorazepam/paroxetine nulls do not predict complete absence of chronic pharmacodynamic effects.

Source 2: original human study

pubmed.ncbi.nlm.nih.gov · Source check Sep 10, 2026

Population: 12 healthy adults.

Short-term dosing had no measured CYP effect; repeated use reduced oral midazolam AUC more than IV AUC.

How this applies: Original source interpreted for eszopiclone only within the stated preparation and endpoint limits.

Not a direct eszopiclone combination trial; original preparation, dose and endpoint limits apply. No exact-pair adverse-event rate or protective spacing established.

Source 3: original human study

pubmed.ncbi.nlm.nih.gov · Source check Sep 10, 2026

Population: 20 healthy men.

Small AUC decrease considered not clinically relevant by authors; no significant Cmax or half-life change.

How this applies: Original source interpreted for eszopiclone only within the stated preparation and endpoint limits.

Not a direct eszopiclone combination trial; original preparation, dose and endpoint limits apply. No exact-pair adverse-event rate or protective spacing established.
