What the direct study showed

After two weeks of one St John’s wort regimen, alprazolam exposure fell by about half and the drug cleared faster. This is direct evidence of altered alprazolam handling, though treatment failure was not measured.

Why the extract matters

A randomized study of a low-hyperforin extract found no clinically relevant alprazolam exposure change. The difference means that findings for one extract cannot be assumed for every St John’s wort product.

Plan starting or stopping

Show the prescriber the extract and dose before changing it. Enzyme effects are not solved by a proven few-hour dosing gap, and you should not increase alprazolam yourself if symptoms worsen.
