The short answer
Avoid grapefruit juice while taking simvastatin. It can substantially raise drug exposure, and the current ZOCOR label links this to a higher risk of muscle toxicity. Taking the juice in the morning and simvastatin later is not a reliable workaround.
What this answer covers
Oral simvastatin and grapefruit juice. Effects from a given juice preparation are not numeric estimates for all Citrus paradisi extracts.
- Supplement or preparation
- grapefruit
- Medication ingredient
- simvastatin
Can one glass matter?
A small crossover study used one 200 mL glass of normal-strength juice daily. Simvastatin and its active metabolite reached several-fold higher exposure after three days. The study measured concentrations rather than muscle injuries, so those increases are not percentages of people who will be harmed.
Why does the label advise avoidance?
Higher exposure can increase the likelihood of statin muscle problems. ZOCOR specifically advises avoiding grapefruit juice. This instruction differs from advice for some other statins, which is why a schedule based on another cholesterol medicine should not be copied.
Does a one-day gap remove the effect?
After a high-juice regimen in another volunteer study, an interaction remained when simvastatin was taken twenty-four hours later. That experiment does not provide a personal waiting-time plan. It explains why simply placing the products a few hours apart is unreliable.
What if I already had grapefruit?
Contact your pharmacist with the amount, product and simvastatin dose. Do not take extra tablets or create your own interruption schedule. Promptly report unexplained muscle pain, tenderness or weakness, especially with fever or feeling unwell; your clinician can assess the need for further testing.

- Exact scope
- Oral simvastatin and grapefruit juice. Effects from a given juice preparation are not numeric estimates for all Citrus paradisi extracts. with Exact RxNorm 36567, IN, simvastatin; ZOCOR oral tablets. No substitution of lovastatin, pravastatin or another statin.
- Medication form and route
- oral
What remains uncertain
- Direct pharmacokinetic evidence plus regulatory management advice. Exact clinical injury rates and equivalent doses for botanical extracts remain unquantified.
Factors that may matter: Exact preparation, dose, other medicines and baseline clinical condition matter..
