The short answer
Fish oil has not been shown to increase bleeding with selinexor in a direct clinical study. Have the product reviewed, especially with low platelets, but a blanket avoidance or spacing rule is not supported by the evidence checked.
What this answer covers
EPA/DHA fish-oil products with oral selinexor. Pure prescription EPA, other omega-3 formulations and food are separate exposures.
- Supplement or preparation
- Fish Oil
- Medication ingredient
- selinexor
Check the prescribed treatment
This review concerns oral selinexor. Its schedule and combination medicines depend on the cancer treatment plan. The drug can cause low platelets, dizziness or confusion, and other medicines and hydration status may contribute. Keep the complete regimen and monitoring plan under the cancer team’s direction.
A large trial measured actual bleeding
In more than 1,500 cardiac-surgery patients, a short EPA/DHA fish-oil regimen did not increase major perioperative bleeding compared with placebo. It also reduced transfusion requirements. This was a clinical bleeding study, not just a platelet test, but it did not involve selinexor.
Do not generalize across all products or patients
The trial’s EPA/DHA mixture and perioperative regimen do not establish the effects of every fish-oil supplement, purified EPA product or long-term high dose. Patients with severe treatment-related thrombocytopenia were not the specific population studied. Read the actual EPA and DHA amounts rather than only total oil weight.
Make changes with the treatment team
Tell the oncology team why you use fish oil and whether it is prescribed. They can assess the benefit against your platelet count, bleeding history and other medicines. No validated time gap was identified for this combination; report unusual bleeding and do not change the cancer-drug dose yourself.
Evidence and practical considerations
Side effects & toxicity
A large original EPA/DHA trial did not increase major bleeding; this limits generic fish-oil bleeding claims without proving safety during the exact cancer treatment.
- Exact scope
- EPA/DHA fish-oil products with oral selinexor. Pure prescription EPA, other omega-3 formulations and food are separate exposures. with RxNorm 2178390 IN selinexor. Exact current oral product: XPOVIO oral selinexor tablets. Salt, formulation and regimen distinctions explicit.
- Medication form and route
- Oral
What remains uncertain
- No exact drug-fish-oil trial. Original EPA/DHA clinical null does not establish universal safety, purified-EPA equivalence or safety in severe thrombocytopenia.
Factors that may matter: Exact oral formulation and supplement preparation; Drug exposure versus clinical outcomes; Dose and duration; Platelet count and neurological symptoms.

Population: Patients prescribed the exact oral formulation.
Thrombocytopenia and neurological toxicity; clinical CYP3A inhibitor and inducer PK null results.
How this applies: RxNorm 2178390 IN selinexor. Exact current oral product: XPOVIO oral selinexor tablets. Salt, formulation and regimen distinctions explicit.
Exact product source, not a direct study of every supplement. Population, active metabolites and formulation scope retained.
Source 2: Original clinical research

Population: 1516 patients undergoing cardiac surgery.
No higher major perioperative bleeding; fewer transfused units.
How this applies: Exact preparation/population/endpoints only; applicability to selinexor independently assessed.
Abstract-only cardiac surgery, not target cancer-drug co-use or all omega-3 forms/durations; not proof of safety in severe thrombocytopenia.
