The short answer

Fish oil is not automatically incompatible with dabigatran etexilate. Does fish oil always increase bleeding?

In the OPERA trial, 1,516 cardiac-surgery patients received mixed EPA/DHA fish oil or placebo. Major perioperative bleeding was not increased with fish oil: the odds ratio was 0.81 with a 95% confidence interval of 0.53 to 1.24. This challenges a blanket statement that fish oil invariably causes excess bleeding.

How close is that evidence to my situation?

The regimen used a short preoperative loading course followed by postoperative fish oil. It was not designed to test chronic fish oil plus dabigatran etexilate as an isolated pair. Dabigatran already increases bleeding risk. Kidney function matters because impaired clearance can increase exposure to active dabigatran. A neutral average result in one surgical population cannot clear every anticoagulated patient or higher-dose formulation.

What should I check on the bottle?

The total fish-oil weight is different from the amount of EPA and DHA. Record both, along with serving size and other ingredients. Prescription pure EPA is not the same intervention as the mixed EPA/DHA trial. Product choice, reason for treatment and the rest of the medication list belong in the assessment.

Should the doses be separated?

No study in the inspected evidence establishes a protective hourly gap or a universal need to avoid fish oil with dabigatran etexilate. Discuss the supplement before starting or changing it, especially with additional aspirin or a prior bleeding problem. Continue prescribed anticoagulation and seek prompt advice for unusual bleeding instead of independently changing its schedule.
