The short answer
Some fiber products can reduce levothyroxine absorption, but the effect varies by preparation. Keep your fiber intake and medicine routine consistent. Current guidance suggests taking levothyroxine at least four hours before wheat-bran fiber; a pharmacist should apply the advice to your exact product.
What this answer covers
Oral levothyroxine with dietary fiber or oral fiber supplements. Wheat bran, psyllium and calcium polycarbophil have different evidence and are not interchangeable exposures.
- Supplement or preparation
- fiber
- Medication ingredient
- levothyroxine
Why is the evidence mixed?
In selected patients needing unusually high thyroid-hormone doses, reducing fiber intake was associated with improved control. Laboratory work also found binding to wheat bran. These observations support a concern for some fiber regimens, but the study was not a randomized comparison of all products.
What about psyllium?
A separate eight-person experiment tested psyllium and calcium polycarbophil. It did not detect malabsorption by the study’s methods. That small, single-dose finding cannot rule out effects during everyday treatment, but it argues against presenting every fiber as equally disruptive.
How long should I separate them?
The 2025 European Thyroid Association table suggests levothyroxine at least four hours before wheat-bran fiber. It does not prove that a three-hour gap works for every supplement. Ask your pharmacist about the actual fiber, any added minerals and the instructions for your thyroid medicine.
Should I stop eating fiber?
A potential interaction calls for a consistent routine and appropriate thyroid follow-up, not an automatic elimination of fiber. If you make a substantial dietary or supplement change, tell the clinician managing your levothyroxine so test results can be interpreted in context.

Absorption & effectiveness

- Exact scope
- Oral levothyroxine with dietary fiber or oral fiber supplements. Wheat bran, psyllium and calcium polycarbophil have different evidence and are not interchangeable exposures. with Levothyroxine active moiety, RxCUI 10582 (IN), supplied as oral levothyroxine sodium tablets. The DailyMed active-ingredient table explicitly connects the sodium salt to levothyroxine. No extrapolation to injectable hormone or another thyroid drug.
- Medication form and route
- oral levothyroxine tablets with oral supplement
What remains uncertain
- Nonrandomized patient observations and a small volunteer experiment differ in population and fiber exposure. Neither establishes a universal minimum interval or a quantitative interaction for every soluble and insoluble fiber.
Factors that may matter: exact product and dose; thyroid condition and treatment target; administration routine.

Population: Patients prescribed oral levothyroxine sodium tablets.
Addresses fasting administration, interfering antacids and food, thyroid monitoring and the risks of over- or undertreatment.
How this applies: Oral levothyroxine with dietary fiber or oral fiber supplements. Wheat bran, psyllium and calcium polycarbophil have different evidence and are not interchangeable exposures. The source supports medication administration and monitoring, not an isolated effect for every supplement.
Does not quantify every supplement interaction; recommendations for these tablets do not establish identical behavior for liquid or injected levothyroxine.

Population: Hypothyroid patients with unexpectedly high levothyroxine requirements.
Thyroid hormone requirements declined with reduced fiber intake; wheat bran adsorbed levothyroxine in vitro.
How this applies: Oral levothyroxine with dietary fiber or oral fiber supplements. Wheat bran, psyllium and calcium polycarbophil have different evidence and are not interchangeable exposures. Apply only the preparation, population and limitations recorded for this source.
Selected patients and nonrandomized withdrawal; does not establish effects or a minimum spacing interval for every fiber preparation.

Population: Eight volunteers studied on three occasions.
Estimated absorption was 89% alone, 86% with polycarbophil and 80% with psyllium; authors did not identify malabsorption detectable by their method.

Small single-dose experiment with wide confidence intervals; not proof of no effect during chronic hypothyroidism treatment.
