The strongest evidence concerns ephedra itself
Federal guidance links ephedrine-containing supplements to high blood pressure, heart attack, stroke and other serious harms. These concerns are especially relevant when torsemide is treating a cardiovascular condition. They do not prove that torsemide amplifies every ephedra effect.
A mixed-product study found pressure and potassium changes
In 16 healthy adults, ephedra-guarana and a multicomponent ephedra/caffeine product raised heart rate and blood pressure and lowered serum potassium. No participant was being studied on torsemide, and caffeine limits attribution to ephedra alone.
Opposite pressure effects do not balance safely
Torsemide lowers blood pressure and can disturb electrolytes. A stimulant product that raises pressure should not be viewed as canceling those effects or as a treatment for fatigue or dizziness. The combined magnitude of risk has not been measured.
Review current use
Do not add an ephedrine-containing supplement to manage weight or energy without discussing it with the treating team. Report current use, including caffeine and other stimulants. Chest pain, fainting or severe new shortness of breath warrants urgent assessment; changing torsemide yourself is not a solution.

Excess diuresis may cause dehydration, hypotension and worsening renal function. Electrolyte abnormalities include hypokalemia; monitor serum electrolytes. Low-dose hypertension studies showed small average potassium decreases, with no universal deficiency. Photosensitivity appears among postmarketing reports.
