A 74-year-old man developed a large thigh hematoma and anemia after adding curcumin 1 g daily. He was taking apixaban 5 mg twice daily and clopidogrel 75 mg daily, with multiple other medicines and a renal-transplant history. He needed transfusion; the report does not isolate curcumin from those other risk factors.
Why is the evidence still uncertain?
Apixaban itself can cause bleeding, and adding an antiplatelet medicine increases that risk. The case did not measure apixaban concentrations or test a repeat curcumin exposure. A separate small healthy-volunteer study of Curcuma longa, including aspirin co-use, did not find clinically important overall coagulation impairment. It did not test apixaban or the same curcumin product.
Are curcumin capsules the same as turmeric food?
No. A concentrated ingredient, whole turmeric powder and a bioavailability-enhanced mixture can give different exposures. A case involving gram-dose curcumin cannot establish the risk from a seasoning portion, nor clear all capsule products. Review the ingredient list, daily amount and any other substances intended to increase absorption.
