The short answer
An increased stimulant effect from bitter orange plus dextromethorphan has not been established. Seville orange juice can alter dextromethorphan handling, but juice, extracts and multi-ingredient stimulants require separate assessment.
What this answer covers
Citrus aurantium juice, fruit extracts and mixed supplements; not grapefruit or purified synephrine as interchangeable exposures.
- Supplement or preparation
- Citrus aurantium
- Medication ingredient
- dextromethorphan
The direct study measured drug handling
A small study gave dextromethorphan with Seville orange juice and measured urinary drug and metabolites. The authors inferred altered bioavailability; they did not establish an increase in clinical stimulant effects.
A different preparation produced a different signal
A separate extract study found no significant change in several drug-metabolizing enzyme probes. Its preparation lacked one relevant citrus enzyme inhibitor. That result cannot cancel the juice finding or prove every extract safe.
Cardiovascular concerns need context
Bitter-orange safety reports often involve other ingredients, and studies of heart rate and blood pressure are inconsistent. Check whether a product also contains caffeine or other stimulants rather than attributing every effect to the botanical name.
Have the actual products reviewed
Show the pharmacist the cough formulation and supplement label before combining them. No protective spacing interval or predictable change in effect has been established. Follow the cough product’s own directions and seek care for concerning symptoms.
Evidence and practical considerations
Side effects & toxicity
Current claim asserts increased stimulant effects; direct juice study assessed pharmacokinetic inference, not that clinical endpoint. Extract probe null and mixed cardiovascular evidence prevent universal botanical claim.
- Exact scope
- Citrus aurantium juice, fruit extracts and mixed supplements; not grapefruit or purified synephrine as interchangeable exposures. with Dextromethorphan3289 IN; oral cough hydrobromide/polistirex formulations, not codeine or other pain opioids; psychiatric combinations explicitly limited.
- Medication form and route
- oral medication; oral supplement
What remains uncertain
- No unmeasured pair incidence, blanket class transfer or arbitrary spacing interval.
Factors that may matter: Preparation; Dose; Other medicines; Organ function.

Population: 11 healthy volunteers.
Urinary parent/metabolite analysis was interpreted by authors as altered bioavailability with both juices and effects persisting through washout.
How this applies: Original abstract only; urinary pharmacokinetic inference, not a clinical stimulant endpoint or every bitter-orange extract. The unusual washout description cannot define a safe gap.
