The short answer

Have a concentrated bitter orange supplement reviewed before using it with levalbuterol. No proven separation interval makes the combination safe.

What this answer covers

Citrus aurantium; standardized extracts, ordinary food and products adulterated with synthetic amines are different exposures.

Supplement or preparation
Citrus aurantium
Medication ingredient
levalbuterol

What the human research says

One small bitter orange extract study increased blood pressure and heart rate. Another study of p-synephrine found no heart-rate increase or extra systolic pressure effect when added to caffeine. Neither tested levalbuterol.

Check the actual product

Concentrated extracts and multi-ingredient weight-loss products differ from food amounts. NCCIH also reports inaccurate labels and synthetic amine adulteration in some supplements. Bring the whole ingredient list to a pharmacist.

Why a time gap is uncertain

The concern is overlapping cardiovascular effects, not a demonstrated binding interaction in the stomach. No original study supplies an interval that prevents it.

Keep the inhaled medicine plan

These findings apply to levalbuterol inhalation products. The nebulizer solution and HFA inhaler have their own instructions. Levalbuterol is the R-enantiomer of albuterol; studies of racemic albuterol cannot automatically supply a dose or risk estimate for this product.

Watch symptoms and asthma control

Report new troublesome tremor, racing heartbeat or dizziness after a supplement change. Fainting, chest pain or severe breathing difficulty needs urgent care. Needing levalbuterol more often than usual is a reason to contact the asthma clinician, not to add more stimulants.

Evidence and practical considerations

Side effects & toxicity

Conditional cardiovascular concern is reasonable but the current blanket need-to-separate-or-avoid claim is not established for every preparation. Original human bitter orange studies differ and did not include levalbuterol.

Exact scope
Citrus aurantium; standardized extracts, ordinary food and products adulterated with synthetic amines are different exposures. with Exact levalbuterol hydrochloride nebulized inhalation solution and tartrate HFA inhalation aerosol. Exact RxNorm IN237159, R-enantiomer of albuterol. Hydrochloride nebulizer and tartrate HFA labels are distinct; racemic albuterol is a comparator, not an interchangeable exact drug.
Medication form and route
inhalation
Timing or duration
Source-specific single-dose, repeated-dose and observational exposures distinguished.

What remains uncertain

  • No exact pair study or validated interval; p-synephrine and synthetic methylsynephrine are not equivalent.

Factors that may matter: preparation and plant part; clinical indication; dose; other medicines; baseline alertness.

Source 3: original human study

pubmed.ncbi.nlm.nih.gov · Source check Sep 10, 2026

Population: Fifteen healthy adults in placebo-controlled crossover.

Blood pressure and heart rate were higher than placebo at some measured hours.

How this applies: Only the named drug, preparation, route and measured endpoints apply.

Single extract and healthy volunteers; no levalbuterol or terbutaline co-use, no clinical event rate.

Source 4: original human study

pubmed.ncbi.nlm.nih.gov · Source check Sep 10, 2026

Population: Sixteen subjects, double-blind comparison at rest.

No heart-rate difference; p-synephrine did not augment caffeine effects on systolic pressure; higher caffeine arms increased systolic pressure.

How this applies: Only the named drug, preparation, route and measured endpoints apply.

Short study at rest with specific preparations; no respiratory drug co-use and no general safety guarantee.

Source 5: official supplement guidance

nccih.nih.gov · Source check Sep 10, 2026

Population: People considering the specified supplement.

Cardiovascular findings are inconsistent; serious events often involve mixtures, and supplement composition can be inaccurate.

How this applies: Supplement-only context; explicit medication and preparation bridge required.

No exact respiratory medication interaction or equivalence to synthetic methylsynephrine.
