Why a change in caffeine can matter
Lithium is cleared mainly by the kidneys. Its label warns that xanthine preparations can increase lithium excretion. An acute study in eight healthy men found increased lithium clearance after caffeine, but it did not study long-term psychiatric treatment or show that caffeine changes intestinal absorption.
What the direct case shows
One man taking sustained-action lithium carbonate reduced a very high caffeine intake from about 1027 to 129 mg per day. Two months later, a routine test found a high lithium level although he had no symptoms. This is a reason for caution, but one case cannot predict your response or establish a safe caffeine cutoff.
Plan substantial changes with your prescriber
Tell your prescriber before sharply increasing or reducing your usual caffeine intake, including energy drinks, coffee and caffeine tablets. Ask whether your lithium level should be checked around the change. Do not use extra caffeine to lower a lithium level or adjust lithium yourself. Taking caffeine a few hours apart has not been shown to prevent this kidney-related effect.
Know when to seek help
New vomiting, diarrhea, worsening tremor, marked drowsiness or trouble coordinating movements can be signs of lithium toxicity. Two months after the caffeine reduction, lithium was 2.1 mmol/L, found incidentally while he remained asymptomatic. Later lithium-dose and caffeine changes coincided with low concentrations and psychiatric relapse.
How this applies: Direct exact-carbonate case supporting caution with substantial caffeine changes; later outcomes are confounded.
Single patient, very high initial caffeine intake, other medicines and subsequent lithium-dose changes. No universal threshold or causal estimate. The original says two months and asymptomatic, unlike a secondary account.
