The short answer
It is not established that berberine changes saxagliptin absorption or clinical effectiveness. Have the pharmacist review the product; enzyme findings from other test drugs do not provide an exact dose adjustment.
What this answer covers
Oral berberine supplement; botanical source and dose must be specified.
- Supplement or preparation
- Berberine
- Medication ingredient
- saxagliptin
What was actually measured
Repeated berberine increased midazolam exposure in a healthy-volunteer enzyme study. This supports a possible metabolic interaction, but the study did not measure saxagliptin or its active breakdown product.
How the medicine label applies
Saxagliptin uses CYP3A4/5 to form an active metabolite. Its label restricts dosing with strong inhibitors, but this probe study does not establish that berberine is a strong inhibitor or predict the combined activity of saxagliptin and its metabolite.
Bring the complete product label
Record the species or ingredient, extract strength, daily amount and start date. Report glucose changes so the clinician can assess supplement effects alongside meals and other medicines.
Keep the prescribed medicine plan
Continue the prescribed saxagliptin dose and review the complete medicine and supplement list with the pharmacist. A new supplement is not a reason to adjust this medicine yourself or invent a separation interval.
Evidence and practical considerations
Absorption & effectiveness
Original absorption/effectiveness claim remains insufficient: human probe findings require an untested medication-specific bridge.
- Exact scope
- Oral berberine supplement; botanical source and dose must be specified. with Exact saxagliptin single-ingredient hydrochloride oral tablets. Exact RxNorm IN857974 saxagliptin; single-ingredient hydrochloride oral tablets. Combination medicines are not assumed equivalent.
- Medication form and route
- oral
- Timing or duration
- Source-specific single-dose, repeated-dose and observational exposures distinguished.
What remains uncertain
- No direct exact-pair human pharmacokinetic or clinical-effect study identified; no safe dose or protective spacing established.
Factors that may matter: preparation and plant part; clinical indication; dose; other medicines; baseline glucose and nutrient status.

Population: People prescribed saxagliptin.
Glucose-dependent DPP4 action; CYP3A4/5 metabolism to active metabolite, dose restriction for strong CYP3A4/5 inhibitors. Botanical classification as strong inhibitor unproven.
How this applies: Exact RxNorm IN857974 saxagliptin; single-ingredient hydrochloride oral tablets. Combination medicines are not assumed equivalent.
No universal supplement dose, interval, event rate or untested formulation equivalence.
Source 2: original human study
pubmed.ncbi.nlm.nih.gov · Source check Sep 11, 2026
Population: Healthy male volunteers in crossover study.
Midazolam exposure increased about 40%; CYP2D6 and CYP2C9 probe measures changed. Other tested probe pharmacokinetics did not significantly change.
How this applies: Only the named drug, preparation, route and measured endpoints apply.
Neither linagliptin nor saxagliptin was studied. This does not establish a strong CYP3A4-inhibitor classification, intestinal absorption change or clinical toxicity.
Source 3: original human study
pubmed.ncbi.nlm.nih.gov · Source check Sep 11, 2026
Population: Adults with newly diagnosed or poorly controlled type 2 diabetes in two pilot cohorts.
Glycemic measures improved; transient gastrointestinal adverse effects were reported.
How this applies: Only the named original drug, preparation, route and endpoint apply; no untested exact medication transfer.
Small pilot studies; no saxagliptin co-use, no exact-pair hypoglycemia estimate or assumed equivalence to every botanical source of berberine.
