The newer study used a combination product
A pilot compared controlled-release acarbose–orlistat plus psyllium husk with conventional orlistat plus placebo fiber, alongside dietary counseling. The primary composite bowel-event score did not differ significantly.
The comparison cannot isolate the pair
The arms differed in acarbose exposure, release formulation and fiber. Some secondary analyses favored the combination, but they do not prove that adding husk improves or worsens tolerance of ordinary acarbose tablets.
An older study tested alternatives, not co-use
A bread-meal study gave acarbose and Plantago mucilage in separate test conditions. It found glucose-response effects but cannot be cited as a combined-use safety trial.
Respond to symptoms without a blanket avoidance rule
Acarbose itself can cause gas and diarrhea. Discuss persistent or severe bowel symptoms and the exact fiber product with the clinician. Follow the husk product’s administration instructions; the available studies do not establish a universal avoidance rule or an exact added-harm rate.

Local intestinal carbohydrate-digestion inhibition; less than2% absorbed as active drug. Alone should not cause hypoglycemia; insulin/sulfonylurea co-use can increase risk. Oral glucose, not sucrose, for appropriate mild-to-moderate lows. Charcoal adsorbents and carbohydrate-splitting enzymes may reduce effect and should not be taken concomitantly.
