Sleepiness is possible with both products

Promethazine can impair alertness. Melatonin can also cause sleepiness, and its effects may last longer in some older people. That supports checking the combination against the actual dose and daily activities.

Feeling sleepy and measured performance are different

A small placebo-controlled study of afternoon melatonin found increased subjective sleepiness after a prolonged task, but no overall adverse effect on its objective driving-test battery. This does not mean every melatonin dose causes measurable impairment.

The COPD study compared the medicines

A cohort study found worse outcomes among promethazine users than melatonin users with COPD. People using both during the defined exposure window were excluded. It therefore cannot be cited as a study showing what happens when the two are taken together.
