The short answer
Melatonin may add to midazolam-related impairment, but a monitored surgical co-use trial did not show a significant sedation difference. The exact preparation, setting and other medicines matter.
What this answer covers
Oral melatonin preparations, not automatically interchangeable doses or release forms.
- Supplement or preparation
- Melatonin
- Medication ingredient
- midazolam
Both agents can affect alertness
Midazolam is a sedative, and a small daytime melatonin study found impaired performance without midazolam. These individual effects support caution but do not quantify this pair’s added risk.
A direct co-use study found no measured increase
In a thyroidectomy study, patients received melatonin or placebo before anesthesia that included IV midazolam, fentanyl and other drugs. Measured sedation did not significantly differ at the reported times. The multi-drug, monitored setting limits interpretation.
Comparison trials answer another question
Studies that give melatonin instead of midazolam do not establish the safety or added effect of taking both. Oral procedural syrup, injection and nasal rescue also have different uses and dosing.
Review use with the treating team
Disclose melatonin before a procedure or during rescue-plan review. Do not add doses or rely on a short gap as protection. Serious breathing difficulty or inability to wake needs emergency help.
Evidence and practical considerations
Side effects & toxicity
Conditional impairment precaution with direct surgical co-use sedation null retained; no exact isolated interaction magnitude or dose/route-independent risk.
- Exact scope
- Oral melatonin preparations, not automatically interchangeable doses or release forms. with Midazolam6960 IN; oral hydrochloride syrup, IV/IM hydrochloride and intranasal midazolam distinguished.
- Medication form and route
- oral medication; intravenous medication; intramuscular medication; intranasal medication; oral supplement
What remains uncertain
- No unmeasured pair incidence, blanket class transfer or arbitrary spacing interval.
Factors that may matter: Preparation; Dose; Other medicines; Organ function.
