The short answer

Use melatonin with lorazepam only within a plan reviewed by your prescriber. Hospitals have studied regimens containing both, but those results do not establish unsupervised home-use safety.

What this answer covers

Oral lorazepam tablets and oral melatonin; enteral ICU protocols and injected lorazepam studies are distinct contexts.

Supplement or preparation
Melatonin
Medication ingredient
lorazepam

Why adding melatonin still needs review

The melatonin product information cautions that benzodiazepine sedation may be enhanced. Lorazepam can impair alertness and coordination, so the actual melatonin product and other sedatives matter.

A hospital protocol included both

An ICU trial used enteral melatonin and hydroxyzine, adding lorazepam when needed. It achieved similar target sedation to an IV strategy, but had more protocol violations and self-extubations. It did not isolate lorazepam plus melatonin or test routine home use.

Do not transfer an injection study to your tablets

A poisoning trial used injected lorazepam when needed. Melatonin shortened delirium, but lorazepam requirements did not differ significantly. This does not establish a tablet dose reduction or a safer dose gap.

Watch for impaired alertness

Lorazepam can affect alertness and coordination. Avoid driving if affected. Seek urgent help for severe difficulty waking or breathing, and tell the prescriber about alcohol, opioids and all other sedating products.

Evidence and practical considerations

Side effects & toxicity

Conditional alertness review is supported; exact-drug co-use protocols exist but do not isolate the pair or quantify chronic outpatient additive sedation.

Exact scope
Oral lorazepam tablets and oral melatonin; enteral ICU protocols and injected lorazepam studies are distinct contexts. with Exact oral lorazepam tablets. Rapid conjugation to lorazepam glucuronide, which has no demonstrable CNS activity in animals. Oral tablet scope; injected lorazepam is not assigned the same supplement evidence.
Medication form and route
oral
Timing or duration
Source-specific single-dose, repeated-dose and observational exposures distinguished.

What remains uncertain

  • ICU studies use multiple drugs, monitoring and special disease states. Optional lorazepam and injected rescue administration cannot be treated as universal oral co-exposure.

Factors that may matter: preparation and plant part; clinical indication; dose; other medicines; baseline alertness.

Research conclusion: supported with conditions · Evidence assessment: low

Population: Patients prescribed oral lorazepam.

Can impair alertness and coordination; other CNS depressants can increase impairment. Opioid combinations have a specific respiratory-depression warning. Dependence and withdrawal require supervised changes. Rapid conjugation to lorazepam glucuronide, which has no demonstrable CNS activity in animals. Oral tablet scope; injected lorazepam is not assigned the same supplement evidence.

How this applies: Exact IN and labeled oral formulation; route, salt and metabolite distinctions retained.

No quantified risk for every herb or universal protective spacing interval. Opioid-specific evidence does not prove equal botanical respiratory risk.

Population: People considering the specified supplement.

Prolonged-release melatonin product information cautions that benzodiazepine sedation may be enhanced. Its measured interaction study involved zolpidem.

How this applies: Preparation-specific class precaution, not a directly measured interaction with lorazepam.

Class precaution and a zolpidem study, not direct evidence for this exact benzodiazepine. Immediate-release and prolonged-release preparations differ.

Population: 348 randomized mechanically ventilated ICU patients.

No significant difference in achieving target light sedation. Enteral protocol had more violations and self-extubations, with a lighter target and lower costs.

How this applies: Exact exposures and preparation limitations apply; no untested dose or route equivalence.

Multi-drug strategy, not isolated lorazepam/melatonin factorial trial. Optional lorazepam means not every enteral patient received the pair. ICU monitoring and rescue drugs limit outpatient transfer.

Source 4: original human study

pubmed.ncbi.nlm.nih.gov · Source check Sep 10, 2026

Population: 56 patients with organophosphate poisoning in intensive care.

Melatonin shortened delirium; lorazepam requirements did not differ significantly.

How this applies: Exact exposures and preparation limitations apply; no untested dose or route equivalence.

Different lorazepam route and acute poisoning context. Does not prove outpatient oral-pair safety or that melatonin reduces lorazepam need.
