What goldenseal changed in volunteers
Fourteen days of a standardized goldenseal extract reduced a CYP2D6 probe measure by about half. The study used debrisoquine, not desipramine.
Why that pathway matters here
A desipramine study found high levels and adverse effects in two poor CYP2D6 metabolizers. The label also warns that inhibiting this pathway can increase tricyclic drug levels.
The result is not a dose calculator
The evidence cannot tell you how much a particular goldenseal product would raise desipramine levels. Whole goldenseal and isolated berberine should not be treated as equivalent.
Agree on monitoring before changing products
Show the prescriber the extract and dose before starting or stopping regular goldenseal. Do not adjust desipramine yourself; symptoms and, when appropriate, drug levels can guide the clinician.
