The label establishes enzyme susceptibility
Dexamethasone is a CYP3A4 substrate. Its label describes increased concentrations with inhibitors and increased metabolism with inducers. This establishes a pathway for interactions, rather than a measured grapefruit-flesh effect.
What the grapefruit experiment showed
A rat study found increased dexamethasone exposure after repeated grapefruit juice. The animals received a high experimental steroid dose in a defined vehicle. Neither that exposure nor the juice preparation establishes a clinical effect from ordinary grapefruit flesh in a person.
Discuss sustained dietary changes
Tell the pharmacist about regular grapefruit intake and all medicines before changing the routine. The evidence does not provide a dependable fruit amount, steroid dose adjustment or number of hours that prevents an interaction. Keep the prescribed dexamethasone plan while obtaining individualized advice.
