The short answer
Echinacea may affect drug-metabolizing enzymes, but its effect on acalabrutinib exposure or effectiveness is not established. Have the exact preparation reviewed and keep the capsule or tablet instructions for your prescribed product.
What this answer covers
Echinacea purpurea preparations with oral acalabrutinib, distinguishing free-base capsules, maleate tablets and active ACP-5862.
- Supplement or preparation
- Echinacea purpurea
- Medication ingredient
- acalabrutinib
Keep capsule and tablet directions separate
CALQUENCE capsules contain acalabrutinib, while the tablets use its maleate salt. Their acid-reducer directions differ: the tablet formulation was designed to allow use with proton-pump inhibitors. This formulation difference does not prove an echinacea interaction, and you should not switch products yourself.
Human echinacea findings differ by experiment
One study found a modest decrease in oral midazolam exposure after echinacea, while a boosted antiviral regimen was unchanged. Another study found opposing intestinal and hepatic CYP3A effects, with no significant change in oral midazolam clearance. These findings do not establish that echinacea is a strong inducer.
An active metabolite complicates the prediction
Acalabrutinib forms the active metabolite ACP-5862. Drug-interaction research considered both compounds, because a change in the parent drug alone does not necessarily describe the same change in total active exposure. No study here measured both during echinacea use.
Review the complete supplement
Show the team the echinacea species, plant part, extract and dose, including blends. No validated spacing interval or automatic acalabrutinib dose change was established. Continue the prescribed formulation and monitoring plan rather than acting on another probe drug’s result.
Evidence and practical considerations
Absorption & effectiveness
Mixed human probe effects do not establish a uniform reduction in acalabrutinib or total active exposure.
- Exact scope
- Echinacea purpurea preparations with oral acalabrutinib, distinguishing free-base capsules, maleate tablets and active ACP-5862. with RxNorm 1986808 IN acalabrutinib. Exact current oral product: CALQUENCE oral acalabrutinib maleate tablets. Salt, formulation and regimen distinctions explicit.
- Medication form and route
- Oral
What remains uncertain
- No exact acalabrutinib-echinacea clinical PK or response study. Modest/mixed other-drug findings cannot establish strong induction or reduced active-moiety exposure.
Factors that may matter: Exact oral formulation and supplement preparation; Drug exposure versus clinical outcomes; Dose and duration; Platelet count and neurological symptoms.
