The short answer

What is known about calcium antacids?

In a randomized crossover study, 16 participants received ofloxacin 400 mg with calcium carbonate antacid 24 hours before, two hours before or two hours after the antibiotic. None of those calcium schedules significantly changed absorption rate or total exposure. The experiment did not test taking the antacid and antibiotic simultaneously, so that scenario remains outside its reassuring result.

Why keep a gap if the study found little change?

The current product label still directs calcium-containing antacids to be at least 2 hours away from ofloxacin in either direction. This is compatible with the intervals studied and avoids extending the finding to an untested simultaneous dose. A different calcium salt or a mixed supplement may require its own assessment rather than a percentage borrowed from another antibiotic.

Do I need to avoid milk?

A separate study in 21 healthy men found that milk did not change ofloxacin absorption rate or extent. Food delayed the peak without changing the total amount absorbed, and the label permits taking tablets without regard to meals. These results do not turn a glass of milk into evidence about every calcium supplement, but they do argue against an indiscriminate dairy restriction for ofloxacin.

What should I check on a calcium bottle?

Identify whether the product is an antacid, a calcium supplement or a combined mineral preparation. Other ingredients such as iron, magnesium or zinc can trigger their own precautions. Give the pharmacist the complete panel and the oral prescription directions so a plan can reflect the actual product rather than treating every calcium exposure as identical.
