The short answer
An absorption problem with fludrocortisone and aluminum is not established by the available direct evidence. If the product is an antacid, ask the pharmacist to apply its specific timing advice.
What this answer covers
Oral fludrocortisone with aluminum. Relevant studies used particular antacid mixtures and other corticosteroids; elemental nutrient entries are not identical to antacid formulations.
- Supplement or preparation
- Aluminum
- Medication ingredient
- fludrocortisone
Which treatment this applies to
This review concerns oral fludrocortisone acetate tablets, the preparation used clinically for fludrocortisone’s mineralocorticoid effects. Adrenal replacement and treatment of orthostatic symptoms require distinct salt and monitoring plans. It does not establish free-base dose equivalence or effects of other routes.
What the studies actually tested
One small study found reduced prednisolone exposure after prednisone with aluminum/magnesium hydroxide mixtures. Other prednisone or prednisolone studies found no significant antacid effect. These are different corticosteroids and mixed antacid products, not direct fludrocortisone trials.
Distinguish an antacid from a nutrient
Tell the pharmacist the compound, formulation and reason for use. An elemental aluminum or magnesium listing does not identify an antacid, its neutralizing capacity or its full ingredient mixture. Findings about a mixture cannot isolate the contribution of each ingredient or establish the same effect for every supplement.
Use the applicable product instructions
A BC Women’s Hospital fludrocortisone handout advises taking it two hours after antacids in its POTS/hypotension program. This is precautionary guidance, not a trial proving the interval prevents this exact interaction. Confirm the schedule with your pharmacist and keep prescribed steroid treatment.
Evidence and practical considerations
Absorption & effectiveness
Other-corticosteroid antacid studies are conflicting and do not prove reduced fludrocortisone absorption with every aluminum product.
- Exact scope
- Oral fludrocortisone with aluminum. Relevant studies used particular antacid mixtures and other corticosteroids; elemental nutrient entries are not identical to antacid formulations. with Exact RxNorm 4452 IN fludrocortisone. Clinical label evidence is oral fludrocortisone acetate 0.1 mg tablets, the ester preparation used for mineralocorticoid effects; this is an explicit active-moiety bridge, not a free-base dose equivalence.
- Medication form and route
- Oral
What remains uncertain
- No direct exact-drug nutrient/antacid PK trial identified. Conflicting other-steroid studies and preparation-specific timing guidance cannot establish a universal absorption reduction.
Factors that may matter: Exact oral preparation and dose; Treatment duration and indication; Blood pressure and electrolyte status; Other medicines.

dailymed.nlm.nih.gov · Source check Sep 10, 2026
Population: Patients prescribed Fludrocortisone acetate oral tablets.
Small oral doses cause sodium retention, increased potassium excretion and BP elevation. Monitoring of salt intake, BP, weight and electrolytes is advised; replacement decisions are individualized.
How this applies: Exact ingredient and specified formulation. Exact RxNorm 4452 IN fludrocortisone. Clinical label evidence is oral fludrocortisone acetate 0.1 mg tablets, the ester preparation used for mineralocorticoid effects; this is an explicit active-moiety bridge, not a free-base dose equivalence.
Acetate formulation bridge does not establish other routes or dose equivalence. Mineralocorticoid replacement is not automatically the systemic glucocorticoid exposure in osteoporosis guidelines.

pubmed.ncbi.nlm.nih.gov · Source check Sep 10, 2026
Population: Five healthy volunteers and twelve patients with chronic active liver disease.
Measured prednisolone peaks and exposure were reduced with mixtures.
How this applies: Applicable only to the stated preparation, route and endpoints; other-drug evidence is indirect.
Abstract-only; prednisone/prednisolone is not fludrocortisone. Mixed salts do not isolate aluminum or magnesium effects, and liver disease changes applicability.

Population: Human prednisone-antacid PK study.
No significant difference in peak, AUC or time to peak.
How this applies: Applicable only to the stated preparation, route and endpoints; other-drug evidence is indirect.
Abstract-only; different corticosteroid and formulations; no proof of exact-pair safety.
Source 4: Original clinical research
pubmed.ncbi.nlm.nih.gov · Source check Sep 10, 2026
Population: Nineteen healthy volunteers; six nonfasting participants had repeat antacid co-use.
Food altered absorption rate but not extent; antacid did not significantly change parameters in the nonfasting subgroup.
How this applies: Applicable only to the stated preparation, route and endpoints; other-drug evidence is indirect.
Abstract-only; small subgroup and different corticosteroid, not a validated fludrocortisone spacing experiment.

bcwomens.ca · Source check Sep 10, 2026
Population: Patients receiving oral fludrocortisone for POTS or neurally mediated hypotension.
Advises potassium/kidney monitoring and taking fludrocortisone two hours after antacids.
How this applies: Exact drug guidance with antacid and indication boundaries.
Clinical handout, not measured absorption or a trial validating the interval; not every magnesium supplement or aluminum exposure.
